Medical Records and Form Completion
Please note, starting on Wednesday, March 26, 2025, Fayetteville Woman’s Care began using an online portal as the primary method for requesting medical records. Using this portal is the most efficient way to request a copy of your records. Requests received in the office will be forwarded to RecordQuest for processing.

Disability Forms – FMLA Forms
Please note, UWH – Fayetteville Woman’s Care began partnering with RecordQuest to process disability and FMLA forms.
- On arrival, you will be asked to complete an intake form to assist with the payment and delivery of your form. Once completed, please leave all forms with front desk staff for processing.
- Forms can also be faxed directly to 910-323-2219.
- You will receive a text or email instructing you to go online to pay for your form. Once payment is received, RecordQuest will begin the completion process.
- Please allow up to 10 business days for RecordQuest to complete the form process.

With this change, there is now a $25 fee per form.
UWH – Fayetteville Woman’s Care has partnered with RecordQuest to collect payments, complete forms, and provide status updates. You may receive communications from RecordQuest during the form completion process. Any information you share is used strictly to fulfill your request.
